A photo to work with. Click on the thumbnail below to download the image I used. This is from a photo CD I have:
1. Take your photo image and make a duplicate (SHIFT+D). Close your original. (I found using a big image and then resizing at the end gave a smoother look to the frame)
2. On the duplicate, starting in the center if you are using the picture provided, its 439 x 217, using the selections tool, feather none, antialias checked, make a circle. I made mine fairly big.
3. Effects--->Geometric Effects--->circle. Do not deselect.
4. Add a drop shadow with the settings shown in the image below. Repeat the drop shadow, but change the vertical and horizontal offset to -3). Deselect. (You may want to change the settings according to the photo you are using)
5. Selections--->Select all. Effects--->Geometric effects--->circle. Do not deselect.
6. Apply Eye Candy 4 Glass - use the clear glass preset, with the following changes (click on the image for full-size and better quality):
Now, see the white specks in the glass frame? These will be eliminated upon resizing, which is why it's best to work with a large picture.
7. Selections--->promote to layer.
8. on your bottom layer, Selections--->Select All. Apply the glass again. Deselect.
9. Resize your picture, merge layers and save. Here is the original one I did, where I discovered these techniques. I would have loved to use it for the tut but it was too small to resize to a decent size and totally get rid of the white specks:

And in this one, I lowered the glass bevel on the circle - gosh, I almost like it better than the original!:
