Optional - font and/or dingbat of your choice
This tutorial was written in PSP 7 but I'm sure it would work in ver 6.x as well
1. Open a new image 600 x 400. Add a new raster layer and name it "lid".
Make sure your rulers are showing (View--->rulers)
2. On this new layer, with your selection tool, antialias checked, feather 0, draw a rectangle from 25, 80 to 375, 180. Flood fill with a colour of your choice. I used #939498. Deselect.
3. Effects--->Geometric Effects --->Skew. Horizontal 25.
4. With the deformation tool, click on the upper center node
and bring down to about 130.
Click on Apply.
5. Selections--->Select all. Touch the image with your magic wand to select it. Apply an inner bevel with the following settings:
Deselect.
6. On bottom layer (Layer1), add new raster layer and name it "box". With the selection tool, same settings, draw a rectangle from 45, 175 to 385, 280 and flood fill with your colour. Apply the same inner bevel and deselect.

7. Go to bottom layer (Layer1) again and add new raster layer, naming it "side". Draw a rectangle from 380, 40 to 480, 280. Flood fill with your colour and deselect. Effects--->Geometric Effects--->Skew. Vertical 45 (your top is going to be cut off but that's okay, we don't need it anyway) Skew again, 25 vertical.
8. Move the side so that the left side is just behind the box (this hides the
lighter left side after the bevel is applied) and the bottom side is in line
with the box's bottom right corner. If it sticks out a bit, don't worry, we'll
get rid of it in a minute or so.
9. Now we are going to get rid of the excess. With your selection tool, same settings, make sure you are still on your side layer, and draw a rectangle from 370, 100 to 490, 134.
Edit--->Clear.
Now draw another rectangle from 428, 115 to 490, 230
Edit--->Clear again.
10. Apply the same inner bevel.
11. If you need to realign the side so that the edges match up, do so now and erase any overlap that is showing, you will probably need to erase the bottom left corner a bit and perhaps a bit on the top.
Tip: Zoom in once or twice to get a better view.
12. Add a new raster layer on top of the layer named "top". Name it "front trim". With your selection tool, same settings, draw a rectangle from 35, 179 to 386, 190.
Flood fill with your colour and apply the inner bevel preset. Do not deselect.
